It is the responsibility of the sports medicine staff to provide the highest quality medical care for the student-athletes at Longwood University. The sports medicine staff consists of designated team physicians, the Longwood University Athletic Training Staff, Longwood University Student Health Center, Longwood University Counseling Center and various other consultants or specialists. The sports medicine staff will provide medical care for the student-athletes in the following manner: pre-participation medical exams, injury prevention, emergency care, and treatment for illnesses, injuries, and or conditions affecting the physical or mental well being of the student-athletes. Yearly Paperwork Each year student-athletes must submit paperwork before he/she can compete in Longwood Athletics. For your convenience, the forms are available on this page in PDF format. Please print the forms from the link below that corresponds to you and return by JULY 15, 2007. Forms may be sent via mail, email, or faxed (434.395.2964). Items emailed or faxed are strongly recommended to be mailed also in case of poor imaging or printout.
